Found this via google, since we didn't actually have a walk through for KoNM, I thought I would post it.
Fight 1: Easy
The spirit plays a very strong defense charm, but it loses all its armor/ward if you play a spirit charm.
Focus on either magic/melee and power through or penetrate its defenses, or sprinkle in some split spirit/damage charms.
Or ignore all that completely and just kill it with banes.
Fight 2: Hard
Inquisitor Woeful is rather famously dangerous for a lot of classes. Armor, willpower, and cleanse are pretty much necessary if you don't want to die horribly. Helm of Repentance, Soul Healing, and Shield of the Brave are all excellent for this fight.
Other than that, he has a respectable 800hp and a slightly low 39 spirit. You can either try to purely tank him for enough turns, with nothing but willpower/armor/cleanses, or else you can try to mix in enough damage or spirit damage to kill him. Your choice will probably depend on your class and charms available; being an Inquisitor myself, I opted to out-spirit him.
Fight 3: Hard
I had more trouble with the Warden than I ever did with Woeful, personally; but your class charms make make this fight easier or harder, comparitively. It's virtually impossible to kill the Warden before the mending spirits are dealt with, and the Warden will repeatedly play Protect charms and pull the focus away from the Mending Spirits.
So, your options:
1. Burst the warden to death in a single hit big enough to get through his armor/ward and dish out 600 damage in a single hit (hard, and unlikely to be reliable).
2. Use "smart focus" (attraction/repulsion charms, nature's claws), "target lowest hp" charms, and/or AoE charms to wipe out the mending spirits first. How hard and/or reliable this is will depend on how many of those charms you have.
3. Wait out the mending spirits while building up CPB. This is the strategy I found to be the most reliable and least RNG-dependent. Basically, just play enough ward to survive until the Mending Spirits exhaust, maybe some Psychic Cry to help them exhaust faster, and a ton of CPB so that once they finally do exhaust, you can easily kill the Warden in two or three hits. I didn't have it when I started running KoNM, but Fury of the Seas is practically made for this strategy. Embroidered Robe is another excellent ward to use here; offensive charms that also give ward, like Blessed Hammer or Slashing Axe "Welcome", are also worth considering.
Fight 4: Moderately Hard
Underworld Bond, which you get at the start of the fight, will give you a one-time +500 to next magic and +500 to next melee, if you live long enough! Unfortunately, you need to live ~45 turns for it take effect.
The usual method is to spam armor/ward/debuffs to survive Baralas, and mix in some dual-damage charms (Sylan Charms and Consecrated Soil are rightly popular) so that, with any luck, one drops towards the end and gets the full +1000 damage from Underworld Bond.
Don't forget, though, that you still need to do another 500 or so damage to Baralas, even *with* the assist from Underworld Bond. Persistent Disease is one popular way of making sure you get enough extra damage done to him over the course of the fight.
The achievement is an achievement for a reason. Unless you have very good armor and very good wards, you will not be able to survive how hard Baralas hits unless you debuff his damage with Feet Binds, Magebanes, Dusk Capes, Power Leaks, and so forth.

Fight 2, you just have to kill him faster than he kills you. Try using some of the 5 willpower weapons that are craftable from skyrift to give you a few extra turns.
Fight 3 there are 2 main tactics. Either use AOE to take out the healers, or use turtle + constant power boost (CPB) to wait out the healers and then crush the skelly in a few rounds (this is the route faen had to go, ward + battledance).
For baralas, you have to survive for 41 turns (usually ends up on turn 83 or so) then you get +600 to next melee and magic from the ghosty thing. Put a few dual damage charms in your deck (10 or so should do it) then hit him with constant debuffs. Feet binds and mage bane poisons are best if you have sap 3, but a mix of debuff and defense might get you there if you have enough life.

Did this work for you, because I am getting killed and armor/ward isn't working

Basic idea worked for me. You need the debuffs to come out early or you will still die. If your health is low you might want to add a few drug inhibitors or Ocean sleep potions.

Magebane is pretty much mandatory for this fight. Shadow whisper is simply not enough...

or you can use sky/earth vita combo. Also drop you magic only charms and replace them with dual magic/melee charms.
The tellium plate is horrible, it is -4 to your magic. Frozen skin is decent. From what i've seen though, you really need to try to get lucky on vita bans...maybe add in a few Decay.

Ok I am getting pissed. I figured I am not ready to get past the 4th fight, but that is ok, I will just continue to the first 3 so I can get all the persistent disease. I have been ion this same one now for 4 or 5 days, and I got one on the first day, but not since. 5 days, my odd should have played out, I would think

It seems my luck has ran out,I don't know how could it happened,two of my alts,one was blocked at the 3rd fight and another at 4rd fight,they usually got passed in 1~3 rounds,but today,the same decks,cost me about 1k AP in total,still failed.WTF!
I decided to give up.....anyway,both of they have got enough insignia already.
